Antibiotic assays: Request cards.

These must include the following information to allow accurate interpretation of the assay result:

  • Patient details
    • Name
    • Hospital number
    • Date of birth
    • Latest creatinine (& date)
  • Drug details
    • Antibiotic name
    • Dose of antibiotic prescribed
    • Frequency of dosing (once daily, twice daily..)
    • Times of doses
    • Indication
  • Sample details
    • Date and time sample taken
    • Type of sample (pre-dose, post-dose, or random sample?)
    • Date and time of last dose of antibiotic

Failure to provide this information may result in the sample being rejected by the lab, or incorrect interpretation of the assay result.

If there is insufficient information on the request card to interpret the result, then the result will simply be reported on PAS or HISS without any interpretation.
